2.            Important Step!  RETURN APPROVAL FAX NOTIFICATION

Simply fill out the short & simple approval notification make sure to provide surgery date & amount financed.  To expedite the payment process, please include any multiple check instructions.  Once completed fax notification to:

3.            RECEIVE FUNDS

Within 24-36 hours of receipt of completed loan forms from the borrower(s) we will disburse payment to your office.  Upon disbursement MedChoice will fax payment notification, to obtain for your records.
